Client Informed Consent Agreement

This Client Informed Consent Agreement (“Agreement”) outlines the terms and conditions for working with Meyer Media LLC, a Pennsylvania Limited Liability Company, and its representative, Cathleen Meyer (“Cat Meyer”). By checking the box at the end of this document, you confirm that you have read, understood, and agree to the following:

Your Relationship with Cat Meyer

You understand that you are solely responsible for your own physical, mental, and emotional well-being, decisions, choices, actions, and results arising from your interactions with Cat Meyer. You agree that Cat Meyer is not liable for any action, inaction, or outcomes related to your work together.

You acknowledge that Cat Meyer is not a licensed mental health professional, and the services provided are not a substitute for counseling, psychotherapy, psychoanalysis, mental or physical health care, or substance abuse treatment. It is your responsibility to seek independent professional support as needed.

If you experience mental distress during or after working with Cat Meyer, it is strongly recommended that you consult with a physician or mental health professional immediately.

You confirm that any techniques learned are not a replacement for professional medical or mental health care.

Eligibility to Work with Cat Meyer

You confirm that you do not have any condition that would make working with Cat Meyer inappropriate, including but not limited to:

- A history of bipolar disorder, schizophrenia, acute or chronic depression, or other mental health disorders.

- Current or recent use of antipsychotics, medication for bipolar disorder, or any mood-altering substances.

- Unresolved drug or steroid abuse.

- Recent thoughts of self-harm, harm to others, or suicide.

- Epilepsy or seizure disorders.

- Any physical, mental, or emotional limitations that would make participation unsafe or inappropriate.

- Current therapy where your therapist advises against working with Cat Meyer.

If you have any doubts or concerns about your suitability for this work, you agree to consult with a healthcare professional before proceeding.

Confidentiality

All information shared with Cat Meyer is kept strictly confidential. However, Cat Meyer reserves the right to disclose information to mental health professionals or law enforcement if there is concern that you or someone else may be at risk of harm. This is solely at Cat Meyer’s discretion and should not be interpreted as a legal obligation to report.

Cat Meyer may occasionally reference statements you make in sessions (without revealing your identity) for educational or promotional purposes unless you specifically request otherwise.

Client Responsibilities

You agree to fully participate in the session and take responsibility for your own progress and results. Cat Meyer does not guarantee any specific outcome.

You commit to being on time, present, and free from the influence of drugs or alcohol during all sessions.

Payment Terms

Deposit is due upon booking. Balance is due on the day of your session.

Rates may change at Cat Meyer’s discretion.
